OCEAN-HP, Healthy Start Program provides high quality health services for pregnant women and to their unborn and
born infants, and/or for parenting women/men and their infants. In the under deserved communities of the South Side
of Chicago in partnership with UI Health/ Mile Square Health Center.

OCEAN-HP - Chicago Partnership for Health Promotion (CPHP)
Chicago Partnership for Health Promotion (CPHP) mission is to provide high quality nutrition education, health promotion and disease prevention services through partnerships, directed at eligible families in Chicago. The CPHP nutrition education team provides services through a wide network of community based organizations with
interventions designed to improve nutrition and reduce health disparities for individuals with nutrition-associated diseases. CPHP provides high quality culturally tailored nutrition education in more than 300 venues reaching every
region of Chicago.
CPHP’s goal is to bring healthy lifestyle messages into communities where Chicagoans live, work, eat and play. CPHP staff encourages Chicago families to make healthier food choices, learn to prepare and consume healthier foods every day and be more physically active. CPHP provides nutrition education through a team of Nutrition educators, drawing on the unique strengths of both academic professionals (often-registered dietitians) and paraprofessional community peereducators.

OCEAN-HP – Mile Square School Based Health Centers
OCEAN-HP, Mile Square School Based Health Centers, are community health centers which are similar to a medical office located inside or in close proximity to a school. The health center staff provide a full range of physical, health education, and behavioral health and wellness services to students so they can avoid health-related absences and achieve greater academic success. School health centers may also provide a wide array of wellness and integrated healthcare services to families, community residents and school staff. Our school health center staff includes licensed, registered and/or certified health professionals who are trained and experienced in community and school health and who have knowledge of health promotion and illness prevention strategies for children, adolescents and adults.
