The Kindness Campaign, The Christopher Watts Initiative and Chicago Urban League will be hosting “2019 Thanksgiving Meal Distribution Day” on Saturday, November 23rd 11:00am – 12:30pm at The Chicago Urban League 4510 S Michigan Ave Chicago, IL 60653.
We will be gifting Thanksgiving meals for selected families to prepare and eat on Thanksgiving Day. Gifts will include a Turkey.
To become a selected family and receive a gift, please visit https://www.surveymonkey.com/r/5NWZRTL and submit a request form. The link will be available until the maximum number of recipients have been reached. If you click the link and the request form is unavailable, that means the maximum number of recipients have been reached. There are not and will not be any extras. All recipients must submit a request form through the link.
The person submitting this form must personally pick-up the gift and provide an ID on Saturday, November 23rd 11am - 12:30pm at The Chicago Urban League 4510 S Michigan Ave Chicago, IL 60653. No gifts will be held for someone that does not come between 11:00am and 12:30pm on Saturday, November 23rd. After 12:30pm, the gift will be given to another family. Someone other than the person submitting this form cannot pick-up the gift. THE PERSON (NAME ON FORM) MUST PICK-UP THE GIFT AND PROVIDE A FORM OF ID ON SATURDAY, NOVEMBER 23RD BETWEEN 11:00AM AND 12:30PM AT 4510 S MICHIGAN AGE CHICAGO, IL 60653.
One gift per household. If an apartment is listed in the address, staff will check city records, United States Postal Service (USPS) and Google Earth to verify that the property is a multi-unit property.
Selected families will be contacted by telephone (on the number provided on the request form) on Wednesday, November 20th. The name listed on the request for must be able to personally pick-up the gift at the 2019 Thanksgiving Meal Distribution
