Southside locations for COVID Vaccines

Trinity United Church of Christ – Imani Village, 901 E. 95^th Street: First dose appointments will be available from April 9^th through May 7^th. This site will serve those who live or work in Chicago.  You will need to present ID indicating your work or home address is in the city limits.  To register, visit https://tinyurl.com/TrinityAdvocateVaccines, and use code 2216876 for this site specifically or call (708) 520-0402 for assistance.   Eligible groups for this location are:

•  65+
  16+ with high-risk medical conditions
  55+ non-healthcare frontline essential worker.

Oak Street Health is providing walk-in vaccinations daily from 9am – 11 am at the 3348 W. 87^th Street, (Kedzie) location.
