In Person Counselor (IPC) Community Health Outreach Worker Job Description

The Greater Auburn Gresham Development Corporation (GAGDC) works to foster and promote revitalization of the low-to-moderate income communities of Auburn Gresham, Englewood, Greater Grand Crossing and West Chatham. Founded in 2001, the GAGDC has worked to transform communities with a focus on housing, education, businesses, technology, hard-to-reach population employment and services and community health initiatives.

 Job Description:

The In-Person Counselor (IPC) will provide community outreach to the Chicago communities of Auburn Gresham, Chatham and Greater Grand Crossing, supporting an awareness campaign to provide educational enrollment referral assistance to uninsured and underinsured families and individuals as part of the Affordable Care Act (ACA) In-Person Counselor Program.This is a Full-Time, Temporary position, August 2013 – June 30, 2014.

 Qualifications:

  • Bachelor degree preferred. High School diploma required at a minimum
  • Complete all In Person Counselor online and in-person trainings/meetings required for In Person Counselor certification
  • Familiarity with Chicago neighborhoods of Auburn Gresham, Chatham and Greater Grand Crossing communities to be served
  • Demonstrated experience in Community outreach, canvassing and organizing is critical. Ties to the community and mechanisms by which to contact and engage the community’s members in health insurance education and enrollment.
  • Excellent verbal and written communication skills with exceptional attention to details
  • Ability to work independently and on group projects, manage multiple tasks, establish priorities and meet deadlines
  • Ability to work with diverse population
  • Bilingual English/Spanish a definite plus
  • Experience with the collection and maintenance of required, relevant records and data
  • Requires non-traditional hours, including limited evening and weekend hours

The IPC Community outreach worker will ( Roles and Responsibilities:):

  • Complete all In Person Counselor online and in-person trainings/meetings required for In Person Counselor certification
  • Leverage their organization’s existing programming and networks within the neighborhood (i.e., churches, other CBOs, social service organizations, local businesses, housing developments) to identify eligible residents and distribute materials
  • Canvass the community/Knock on doors, as part of outreach campaign
  • Organize/hold outreach and educational events focusing on target audience of the uninsured and those who would benefit from enrolling in the Health Insurance Marketplace (exchange) as part of the Affordable Care Act.
  • Attend community events (health fairs, etc.) to outreach to target audience
  • Target areas in the community where large numbers of eligible people may gather (churches, block club meetings, parks, local sporting events, job fairs, places where young adults congregate, CTA stations and bus stops, libraries, etc.)
  • Promote information via neighborhood portals/websites, neighborhood press
  • Make referrals as needed to other IPC organizations (i.e. bilingual support)
  • Follow up with residents to ensure they’ve initiated the application process
  • Be available to answer questions, explain health coverage options available
  • Stay abreast of best practices in outreach in their neighborhood and beyond and exchange best practices with LISC’s other subgrantee organizations
  • Conduct some activities on evenings and weekends to accommodate the greatest number of residents possible
  • Connect residents to other health-related information and resources that may surface during interactions with residents
  • All applicants must possess a valid driver's license, car insurance and a reliable car.
